Please note that all participants must fill out and sign the medical questionnaire, either on paper or online, after the booking is confirmed.

A yes answer in Section 1 means that there are underlying medical conditions unsuitable for this activity, and for your safety, we cannot offer the activity to you.

A yes answer in Section 2 means that you should check with your doctor to make sure that this activity is appropriate for you, and we need our physician approval form (page 2) to be signed off by your doctor.

All participants over 60 years of age must have their doctor sign the approval form.
